2019 Tax Organizer
Use the organizer to help gather your information and forms, and complete the tax organizer to the best of your ability. Give the organizer to your PPG Partners CPA or professional, along with all applicable forms and documents needed to complete your 2019 tax return. Note that this is an all-inclusive organizer, and many of the pages will most likely not apply to you.

Instructions for Form I-9
(Version Current, according to, "Until further notice, employers should continue using the Form I-9 posted here [revision date 7/17/17 N]") Employers must complete Form I-9 to document verification of the identity and employment authorization of each new employee (both citizen and noncitizen) to work in the United States. Be sure to always use the most current version of Form I-9.
*If you use the smart version of Form I-9, the form must still be printed out and signed by the employee and the employer representative.

Form W-4, Employee's Withholding Allowance Certificate
(Updated 2019) Complete Form W-4 so that your employer can withhold the correct federal income tax from your pay. Consider completing a new Form W-4 each year and when your personal or financial situation changes. (Use the Withholding Calculator to help you determine what to withhold. See Withholding Calculator Frequently Asked Questions for more information.)
Note that there will be a new 2020 Form W-4 that employeers must use beginning January 1, 2020. It is currently in draft form. Please see for the most current form.

Form W-9, Request for Taxpayer Identification Number and Certification
(Version Current for 2019) A person or company who is required to file an information return with the IRS must obtain the correct taxpayer identification number (TIN) for each vendor or person to include in the return.

Form 945, Annual Return of Withheld Federal Income Tax
(Tax Year 2018) Use for any withholding reported on Forms 1099 and W-2G.

Form 2848, Power of Attorney and Declaration of Representative
(Version Current for 2019) Use Form 2848 to authorize an individual to represent you before the IRS. Your authorization of a qualifying representative will also allow that individual to receive and inspect your confidential tax information.

Form 8822, Change of Address
(Version Current for 2019) After moving into a new residence, you can use Form 8822 to inform the Internal Revenue Service (IRS) of your change of address. Only use this form for changes that affect individual, gift, estate, or generation-skipping transfer tax returns.

"EEO is the Law" Poster
(Version Current for 2019) The law requires an employer to post a notice describing the Federal laws prohibiting job discrimination based on race, color, sex, national origin, religion, age, equal pay, disability or genetic information.



Form WT-4, Employee's WI Withholding Exemption Certificate/New Hire Reporting
(Updated October 2019) Every employee is required to file a completed Form WT-4 with each of his or her employers. (Use the Withholding Worksheet to help you determine what to withhold.)

Form A-222, WI Dept. of Revenue Power of Attorney
(Updated 2019) Form A-222, Power of Attorney, or similar written authorization, executed by the taxpayer is required by the WDR in order for the taxpayer’s representative to perform certain acts on behalf of the taxpayer and to receive and inspect certain tax information.



Illinois New Hire Reporting Form
(Version current for 2019) Employers must report each new hire within 20 days.

Form IL-W-4, Employee's and other Payee's Illinois Withholding Allowance Certificate
(Version current for 2019) Every employee is required to file a completed Form IL-W-4 with each of his or her employers unless the Employee claims the same number of withholding exemptions for Illinois withholding as for federal withholding.

IL-2848, Power of Attorney
(Version current for 2019) Tax practitioners, preparers, etc., are required to have an IL-2848, Power of Attorney, on file with the IL Dept. of Revenue before IDR representatives can discuss client information with them. 



Direct Deposit Authorization
A summary of the terms of agreement between a company employee and PPG Partners, LLC for electronic transfer of funds from the PPG payroll clearing account directly to the employee's personal bank account.
