Instructions for Form I-9
(Updated 07/17/17) You must use the new Form I-9 beginning September 18, 2017. Employers must complete Form I-9 to document verification of the identity and employment authorization of each new employee (both citizen and noncitizen) to work in the United States.
*If you use the smart version of Form I-9, the form must still be printed out and signed by the employee and the employer representative.

Form W-4, Employee's Withholding Allowance Certificate
(Updated 2017) Complete Form W-4 so that your employer can withhold the correct federal income tax from your pay. Consider completing a new Form W-4 each year and when your personal or financial situation changes. (Use the Withholding Calculator to help you determine what to withhold.)

Form W-9, Request for Taxpayer Identification Number and Certification
(Version Current for 2017) A person or company who is required to file an information return with the IRS must obtain the correct taxpayer identification number (TIN) for each vendor or person to include in the return.

Form 945, Annual Return of Withheld Federal Income Tax
(Tax Year 2016) Use for any withholding reported on Forms 1099 and W-2G.

Form 2848, Power of Attorney and Declaration of Representative
(Version Current for 2017) Use Form 2848 to authorize an individual to represent you before the IRS. Your authorization of a qualifying representative will also allow that individual to receive and inspect your confidential tax information.

(Version Current for 2016) The law requires an employer to post a notice describing the Federal laws prohibiting job discrimination based on race, color, sex, national origin, religion, age, equal pay, disability or genetic information.

Form WT-4, Employee's WI Withholding Exemption Certificate/New Hire Reporting
(Version Current for 2017) Every employee is required to file a completed Form WT-4 with each of his or her employers unless the Employee claims the same number of withholding exemptions for Wisconsin withholding as for federal withholding.

Form A-222, WI Dept. of Revenue Power of Attorney
(Version Current for 2017) Form A-222, Power of Attorney, or similar written authorization, executed by the taxpayer is required by the WDR in order for the taxpayer’s representative to perform certain acts on behalf of the taxpayer and to receive and inspect certain tax information.

 

Illinois

Illinois New Hire Reporting Form
(Version current for 2017) Employers must report each new hire within 20 days.

Form IL-W-4, Employee's and other Payee's Illinois Withholding Allowance Certificate

IL-2848, Power of Attorney
