Federal

Form I-9, Employment Eligibility Verification - PDF version (fillable PDF)
Instructions for Form I-9
(Edition 08/01/23 - Expires 07/31/2026) Employers must complete Form I-9 to document verification of the identity and employment authorization of each new employee (both citizen and noncitizen) to work in the United States. Be sure to always use the most current version of Form I-9.

Form W-4, Employee's Withholding Allowance Certificate
(Tax Year 2023) Complete Form W-4 so that your employer can withhold the correct federal income tax from your pay. Consider completing a new Form W-4 each year and when your personal or financial situation changes.

Form W-9, Request for Taxpayer Identification Number and Certification
(Rev. October 2018) A person or company who is required to file an information return with the IRS must obtain the correct taxpayer identification number (TIN) for each vendor or person to include in the return.

Form 945, Annual Return of Withheld Federal Income Tax
(Tax Year 2022) Use for any withholding reported on Forms 1099 and W-2G.

Form 2848, Power of Attorney and Declaration of Representative
(Rev. January 2021) Use Form 2848 to authorize an individual to represent you before the IRS. Your authorization of a qualifying representative will also allow that individual to receive and inspect your confidential tax information.

Form 8822, Change of Address
(Rev. February 2021) After moving into a new residence, you can use Form 8822 to inform the Internal Revenue Service (IRS) of your change of address. Only use this form for changes that affect individual, gift, estate, or generation-skipping transfer tax returns.

Wisconsin

Form WT-4, Employee's WI Withholding Exemption Certificate/New Hire Reporting
(Rev. December 2021) Every employee is required to file a completed Form WT-4 with each of his or her employers. Note that employers must require new employees, and any employees who want to change their withholding, to provide Form WT-4; they can no longer use Form W-4 for their Wisconsin withholding.

Form A-222, WI Dept. of Revenue Power of Attorney
(Rev. October 2021) Form A-222, Power of Attorney, or similar written authorization, executed by the taxpayer is required by the WDR in order for the taxpayer’s representative to perform certain acts on behalf of the taxpayer and to receive and inspect certain tax information.


Illinois

Illinois New Hire Reporting Form
(Version current for 2022) Employers must report each new hire within 20 days.

Form IL-W-4, Employee's and other Payee's Illinois Withholding Allowance Certificate
(Rev. May 2020) Every employee is required to file a completed Form IL-W-4 with each of his or her employers unless the Employee claims the same number of withholding exemptions for Illinois withholding as for federal withholding.

IL-2848, Power of Attorney
(Rev. March 2022) Tax practitioners, preparers, etc., are required to have an IL-2848, Power of Attorney, on file with the IL Dept. of Revenue before IDR representatives can discuss client information with them.
